We may use and disclose your PHI for the following purposes related to providing your IV hydration and injection services. These are the three main categories defined by HIPAA:

A. Treatment (T)

We may use your PHI to provide, coordinate, and manage your health care and related services. This includes coordinating or consulting with your Medical Director, Registered Nurses (RNs) providing your service, and potentially your primary care physician to ensure safe and effective care.

  • Example: An RN reviewing your medical history before administering a vitamin infusion.

B. Payment (P)

We may use and disclose your PHI to obtain payment for the health care services we provide to you.

  • Example: Sharing basic service details on an invoice or superbill for you to submit to your insurance (since most IV services are cash-pay, this primarily involves billing you).

C. Healthcare Operations (O)

We may use and disclose your PHI for the necessary operations of Living Drip Wellness Spa. These activities include, but are not limited to, quality assessment, training, auditing, and business planning.

  • Example: Reviewing treatment records to improve the quality of our service protocols or for employee performance evaluation.

4. Disclosures That Require Your Written Authorization

We will not use or disclose your PHI for purposes other than TPO (Treatment, Payment, Operations) without your specific written Authorization.

  • Marketing: We will obtain your authorization before using your PHI for marketing purposes (e.g., publishing a testimonial or a picture with your service). However, we may communicate with you about health-related products or services we offer.

  • Sale of PHI: We are strictly prohibited from selling your PHI without your express written authorization.

5. Your Rights Regarding Your PHI

As a client of Living Drip Wellness Spa, you have the following rights regarding your PHI:

  1. Right to Inspect and Copy: You have the right to inspect and obtain a copy of the PHI we maintain about you. We may charge a reasonable fee for copying costs.

  2. Right to Amend: If you feel that PHI we have about you is incorrect or incomplete, you may ask us to amend the information.

  3. Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we have made of your PHI.

  4. Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you for treatment, payment, or healthcare operations. We are not required to agree to your request, but if we do, we will comply.

  5. Right to Request Confidential Communications: You have the right to request that we communicate with you about medical matters in a certain way or at a certain location (e.g., calling only your mobile phone, not your home phone).

  6. Right to a Paper Copy of This Notice: You have the right to obtain a paper copy of this notice upon request, even if you have agreed to receive the notice electronically.

6. Our Duties

  • We are required by law to provide you with this Notice of our duties and privacy practices.

  • We reserve the right to change the terms of this notice, and any revisions will apply to all PHI we maintain. We will post the revised notice on our website and make copies available upon request.

  • We will notify you promptly if a breach occurs that may have compromised the privacy or security of your PHI.
